Write a report to your supervisior at work or to the campus safety committee at your college fully describing the circumstances surrounding an accident or injury youve expirienced at work or at college and the results of that mishap. include suggestions about how similar situations might be avoided in the future. Use the memo report format and include visual if appropriate.

Go over pages 81-117 and concentrate on incident reports and progress reports.
No matter where you work, the unexpected frequently occurs. There might be an accident, equipment breakdown, delivery delays, cost overrun and frequently personnel problems. Even in schools these incidents happen. On these occasions you need to write them up, you need to prepare an incident report. An effective incident report should cover the following points:

· What happened – facts, think of the 5 W’s ( not opinionated)
· What caused it – try a detailed chronological order
· What were the results – injuries, losses, costs, damages
· What can be done to prevent a repetition – evaluation
