Write a three to four (3-4) page paper, along with a ten (10) question survey, in which you:
1. Revise Assignment 2 as needed based on feedback provided by your professor. (Revision section is not included in the page count.)
(Note: Provide Parts 1-3 (Background, Directions, and Survey) on the survey for participants to read and complete. Also include Parts 1-3 in your paper. The survey should be sent to at least
twenty (20) participants. Your goal is to obtain ten (10) or more responses. You may use students in the class as well as colleagues and friends. Ask them to answer based on the information you
provided them in Part 1.)
Part 1: Background
2. Provide a brief background of the assessment project by describing at least one (1) purpose and at least two (2) types of information the survey is designed to collect. Part 2: Directions
3. Provide clear directions for taking and submitting the survey within a specified time range. Inform the participants that the responses will be confidential and will be used in aggregate form.
(Note: The directions will go at the top of the survey. The directions may also need to be in the email to the participants. Make sure to email each participant separately or use BCC or blind
carbon copy to undisclosed recipients so that you do not give email addresses to other participants. )
Part 3: Survey
Develop a survey with ten (10) closed-ended, multiple-choice questions, using a Likert Scale, that ask the stakeholders and participants about the needs and goals of the assessment project.
(Note: In a 5point Likert Scale, 1 is very low, 2 is low, 3 is neutral or undecided, 4 is high, and 5 is very high.)
(Note: Guidelines for Interview Assignments at Strayer University:
(1) Before students engage in an interview assignment, they must adhere to these Guidelines for Interview Assignments at Strayer University:
(a) The purpose of the assignment is to train students in interview/research methods, not to develop or contribute to research that can be applied to situations beyond that studied or that is
created to share with others beyond the local setting.
(b) No publication of projects containing the results of or information from the interviews is allowed, including publication in social media, blogs or the Internet generally. The interview findings
may only be shared in the class.
(2) When presenting the results of their interviews to their instructor and / or classmates, students may not disclose personally identifiable information about an interviewee, unless they have
received written permission from the interviewee as verified by their instructor. (3) All interviewees must be at least 18 years old.)
Your assignment must follow these formatting requirements:
• Be typed, double spaced, using Times New Roman font (size 12), with one-inch margins on all sides; citations and references must follow APA. The survey should be single spaced with double
spacing between questions. Check with your professor for any additional instructions.
• Include a cover page containing the title of the assignment, the student’s name, the professor’s name, the course title, and the date. The cover page, revision of the previous assignment, and
the reference page are not included in the required assignment page length.
