Program Evaluation Plan
This document should include two sections. The first is the formative evaluation. In this section, discuss how and when you would conduct the formative evaluation(s) and specific questions you would ask. Include information on how you may incorporate the responses into the course/program. The second section is the summative evaluation. In this section discuss how you would conduct the summative evaluation and specific questions you would ask. Include information on how these responses may affect future planning and who it would be important to share this information with.
The evaluation should include:
• The formative evaluation for your course; a description of how and when you would conduct the evaluation; and reflection on how you might incorporate the information in your instruction
• The summative evaluation for your course; a description of how and when you would conduct the evaluation; reflection on how you might incorporate the information in your instruction
• Who the stakeholders are for your instructional activity and how will you include them
Scoring: 50 pts
The formative evaluation for your course – 15 points
A description of how and when you would conduct the evaluation – 5 points
Reflection on how you might incorporate the information in your instruction – 4 points
The summative evaluation for your course – 15 points
A description of how and when you would conduct the evaluation – 5 points
Reflection on how you might incorporate the information in your instruction – 4 points
Who are the stake holders for your instructional activity and how will you include them – 2 points
