Review the Healthy People 2020 Determinants of Health (see attached documents), select two, and determine how these would affect one of the following population groups: infants, adolescents, adults with disabilities, OR the frail elderly.

2. Focus on a health issue relevant to rural populations. You plan to develop an innovative primary prevention program to address this health issue in a rural community.
a. Discuss how you would assess the community, including how you would gain community support for your prevention program.
