1. Write your introduction and paper rough draft.
a. Write in your own words, do not copy from your sources.
b. Grammar and sentence structure will be reviewed.
2. Select 40 (no more, no less) medical terms from the introduction and paper.
3. Divide the paper into 5 parts as follows:
a. Part 1: Face Sheet (Start on a separate sheet of paper)
i. Type “I. Face Sheet” in the left upper corner.
ii. Type the topic of the paper in the middle of the page.
iii. Type your name, class and session, and date in the right lower corner.
b. Part 2: Introduction (Start on a separate sheet of paper)
i. Type “II. Introduction” in the upper left corner.
ii. Type only two to three sentences describing your topic.
c. Part 3: Medical Terms (Start on a separate sheet of paper)
i. Type “III. Medical Terms” in the left upper corner.
ii. Individually identify the 40 terms previously selected.
1. Place terms in alphabetical order.
2. Number each term.
3. Define the term selected in one to two sentences.
d. Part 4: Clinical Paper (Start on a separate sheet of paper)
i. Paper is to be typed and double spaced.
ii. A minimum of three full typed pages is required.
1. Paper is not to exceed four typed pages.
2. Diagrams/pictures, tables, etc. are not part of the three page criteria, but do count as extra credit.
3. Margins to be no more than one inch on ALL sides (top, bottom, left and right).
iii. Type “IV. Clinical Paper” in the left upper corner at the margin.
1. Double space and start the clinical paper on this line.
2. Underline selected medical terms chosen in part 3, the first time it appears in the introduction and paper only.
e. Part 5: Source (Start on a separate sheet of paper)
i. Type “V. Source” in the left upper corner.
ii. List only the names of the books where you obtained your information.
iii. No bibliography or footnotes please!
