From the point of view of the captain I think the problem was ignoring BP engineers about the wrong casing. As well as Halliburton cement warnings because of the extra time to do it properly. BP was already behind 43 days at a cost of more than $21 million dollars.

I assume the issues related to the leaking preventer, wrong cement casing and “centralizers” were kept of Transocean and the captain. If this information was presented to them they could shut down drilling until it is fixed making them more money and keeping the rig safe.

Having a Plan of action for emergencies
Chain of Command in place ( know who is in charge)
Alert Coast guard and shore of any explosion
Three alternative solutions
MMS shuts down testing completely until the preventer is repaired.
Do not ignore Halliburton use the procedures laid out for proper cementing and install the additional “centralizers”.
Stop drilling and install the proper casing for the well as warned by BP engineers
