    Evidenced-Based Practice
    1) Select a research article or clinical practice guidelines from a comprehensive evidenced based database that relates to hospice or home health care.
    2) In a 250-500-word paper, summarize the research article or clinical practice guidelines . Discuss the application of the research article or clinical practice guidelines with preceptor or other agency staff.
    Example:
    a) Summary – Factors that reduce rehospitalization rates for home health agencies
    b) Application – Discuss with preceptor or agency manager, agency rehospitalization rates? Do they track reasons for rehospitalization? Have they identified preventive measures?
    3) Use standard paper format in APA style, including an introduction, conclusion, and title page. An abstract is not required. Cite in-text and in the Reference section.
    4) Attach the article, saved as pdf file or working link to the article. Assignment will not be graded without access to article. Late deduction will apply if article is not submitted with assignment.
    5) Refer to the Evidence-Based Practice Grading Rubric located in assignment dropbox
    6) Submit the assignment to the Turnitin drop box. Submit the assignment to the instructor by the end of Module 5.
