Cause-and-Effect (Fishbone) Diagram

    Make sure that the fishbone diagram you develop for this assignment aligns with the topic or opportunity for improvement you select for the project.
    Overview of Instructions:
    •Select the problem or “effect” relevant to your clinical micro-system and your project for analysis.
    •Consult with the members of your clinical micro-system team to gather as much information as possible on the possible causes or drivers of the problem and how they should be prioritized (i.e., which one/ones should be addressed first).
    •Categorize the information using the categories described in the Nelson or Q Solutions texts or create your own categories. Make sure your categories are logical and relevant to the problem you are working on and that the “causes” in each category are aligned with their respective categories. For example, you would not want to include “lack of staff training” in a category relating to facilities or equipment.
    •Create a fishbone diagram depicting the relationship between the causes and the problem (or effect).
    •Evaluate/analyze and summarize the results to be read in an audio presentation along with the diagram.

    Cover the following points in your discussion
    1.The problem (or effect) you have selected for analysis and a brief description of how/why it is a problem for the staff/patients of your clinical micro-system.
    B. Which of the Institute of Medicine Six Aims of Improvement does the problem/effect fall under?
    2.The major categories of causes of the problem (i.e., how or why you selected and organized your analysis using the major categories you have opted to use)
    3.Which one of the “causes” you and/or the members of your proposed QI team have selected to work on first (i.e., this should align with or relate to the process you plan to improve for the major project assignment) and how/why it was selected as the “priority for action”
