    George manages very busy Coles supermarket at the Belconnen area, which caters for 8,000 customers per day. At approximately 10.05am on 15 January 2013 a customer dropped a glass jar of honey, causing the jar to smash and the honey to spill over the floor. The customer did not tell any of the supermarket staff of the accident and quickly left the store.

    The practice of the store staff is to clean up any spilt liquids as soon as they are discovered, and further to check for such spillages on opening at 6am and then at 10am, 2pm and 6pm.

    At 11.00 am on the same day Mary entered the supermarket with an elderly man, John.

    Mary is a full-time carer responsible for John, who has poor vision, and so Mary always accompanies him on his walks and shopping trips. Whilst walking down one of the supermarket aisles, Mary stopped to talk with a friend. While Mary talked with her friend, John continued walking down the aisle of the supermarket between the rows of goods for sale.

    He trod on the broken glass of the honey jar and severely injured his foot.
