    Create a three-column table for the Six Aims for Improvement, adopted from the Institute of Medicine’s Crossing the Quality Chasm (2001). Using the failed system you described in the previous competency (304.1), fill in examples of when each of the six criteria was met or not met (or could be improved). Column one should list the six aims and their definitions, column two should list instances when the aim was met, column three should be instances when the aim was not met.
