Increasingly, health professionals will need to work with clients in a more interactive, collaborative style. In this assignment you will select a population, and research services this population needs and services currently available for this population within your community. In Nursing 415: Community and Public Health Nursing Practicum you will educate a selected population. You can use the same population you select for this assignment in NURS 415.
1. Consider the community or city where you live in terms of its similarity to Healthy Communities and Cities.
a. Identify at least 3 characteristics of your community or city that are consistent with the precepts of Healthy Communities and Cities.
b. What partnerships exist among public, private, and nonprofit groups in your community or city?
c. What barriers or obstacles do you observe that keep your community or city from becoming one of the Healthy Communities and Cities?
d. What strengths exist in your community or city to promote the Healthy Communities and Cities concepts?

2. Identify 4 specific ways in which public or community health nurses can initiate Healthy Communities and Cities activities.

3. Examine health statistics and demographic data to identify rates of poverty, homelessness, abuse, etc. in the area surrounding your community or city.
a. What services are available through the federal government, the state, and the local community?
b. Survey the surrounding area to identify community resources, soup kitchens, health centers, food banks, and clothing distribution centers for these populations.
c. How do persons who need these services access them? Give specific examples.
d. Discuss eligibility requirements for available services and the methods used to evaluate the services.

4. After examining populations in your area, select a population you are interested in within your community or city.
a. Identify 5 agencies in the surrounding area that provide health-related services to this selected population.
b. For each agency identified in your community or city, discuss:
i. What specific services are provided?
ii. Who is eligible for services?
iii. How service is initiated
iv. The cost of the services

5. Review a copy of Health People 2020 and find Quality Assurance (QA) goals for care in the community. List these QA goals. Describe ways these goals could be met in your community or city.

6. Select a health situation that you have identified in your community or city. Using the multilevel framework for each of the 4 levels of client (individual, family, aggregate, and community) describe:
a. Illness care
b. Illness and disease prevention
c. Health promotion care
